You can update your address online at UC for Yourself. Just log on and click "Update Your Personal Information". OR if you prefer, you can download a Personal Data Form (PDF) and complete it with your new information. Sign the bottom of the form and send it to mail code 0737.
If you changed your name, download the Personal Data Form (PDF) and complete the appropriate sections of the form, then sign and date the bottom and
send a copy to our office, mail code 0737. You must also include an updated
photocopy of your Social Security Card with your legally updated name. This must be done before your name can be changed in our Payroll System, which affects what is printed on your pay stubs.
You can now view and print an official copy from https://atyourserviceonline.ucop.edu/ayso/. Enter your username and password. New users: Click New User to set up a username. Prior users: Click Forgot your username or password? to reset these. Click W-2 under the Income and Taxes heading in the top right corner of your screen. Click the year you want to view and print the page.

I would like to
start direct deposit
Before you cancel your old account, fill out a new Wage Distribution Request form to set up the new account OR cancel your existing direct deposit. If you fail to do this first and the University pays you through the cancelled account, you will not receive payment on payday. You will have to be reissued a paper check which can take anywhere from 2 to 7 days. The check can be picked up at the Payroll Office at 10280 North Torrey Pines Road, Suite 305, La Jolla, Ca 92037 OR we can request that the check be mailed to the Business Office located on campus in the Ivy Building and you can pick it up there.
If you have not changed accounts yet, fill out a new Wage Distribution Request form and attach a voided check for your new account. Direct deposit to your old account will automatically be cancelled once your new form is processed. If you are with the same bank and just changing accounts, the direct deposit change should take effect immediately. If you are moving your direct deposit to a different bank you will receive a paper check until the change takes effect. Again, this takes approximately one to two pay periods to become effective.
If you did not receive your timesheet you may download a blank Absence Reporting Slip. Complete the timesheet and fax it to 858-822-0014.
If you did not receive your paycheck, call (858) 534-0233 in the Psychiatry Business Office to confirm that a check was issued. If a check was issued and it did not reach you, you may request a stop payment and a new check. The Business Office will fax you the necessary Stop Payment Request form which requires your signature. If you fax the signed form back by 2 pm, a replacement check can usually be issued the following business day after 3 pm. You may change the number of allowances or the status you’re claiming online at At Your Service. Just log on and click Tax Withholdings under the Income and Taxes heading in the top right corner of your screen. OR if you prefer, you can download a W-4 form and complete it with your new information. Sign the bottom of the form and send it to mail code 0737. If we receive the change prior to the payroll deadline it will be effective for the following pay period.
You have 31 days from the date of a life-changing event to enroll or add dependents to your benefits. Examples of life-changing events include marriage, the birth of a child, the loss of a spouse's insurance coverage, and specific other events.
If you miss the period of eligibility you will be automatically enrolled in Core Medical, Delta Dental, and Vision. To add any dependents or make any changes you will have to wait until Open Enrollment, which takes place once a year during the month November. Changes made during Open Enrollment will be effective January 1st of the following year.
You may enroll for UCSD benefits online at At Your Service. On the right side of the screen, click on "Your Benefits Online". Log on and follow the instructions to enroll.
