When a postdoctoral scholar will be terminating his/her appointment early for any reason, a revised, typed Postdoctoral Scholar Appointment Form must be submitted to reflect the new end date. On the revised form, the "Amendment" box must be checked and the early termination date must be entered as the new end date of the appointment. The rest of the form should be completed as usual, signed by the postdoctoral scholar, the mentor, and the department chair, and the original copy mailed to Renee Hatch at mail code 0603.
